myhealth Essentials
myhealth essentials is a basic level health assessment which measures the key health indicators for working people. Physical measurements are taken and recorded for the individual to then enter into an online health portal for ongoing self-management of their health.
Who is it for?
This is a cost effective and time efficient health program for large workforces especially those spread across regional and remote locations. Measurements can be taken by any health professional including your in-house site nurses. All that is required is access to the internet to allow the individual to create their own personal health centre - myhealth.
What we offer specifically ...
myhealth essentials is comprised of three components:
1. Biometric Assessment
Biometric measurements requiring a physical assessment can be completed by your own employee (such as site nurse), the individuals GP, a current health professional who is onsite regularly or a Ford Health consultant. This assessment includes:
- Fingerprick blood tests for cholesterol, HDL cholesterol and glucose
- Physical measurements of height, weight, waist & hip measurement, blood pressure
- Can be conducted in the workplace
- Duration of 15 minutes
2. Online Health Assessment
The online health assessment is completed by the individual. This includes:
- A questionnaire for identification of key work, lifestyle, medical, physical and psychological health risks
- A Personal Health report with test results, a prioritised list of identified health risks, recommended action plan
- GP referral letter
- Access to health information and resources
3. Online Health Coaching
Through myhealth, the individual can:
- Set health goals
- Access a 12 month online behavioural change program
- Use health tools and resources including e-Learning
- Track their key health indicators ie weight, blood pressure, glucose
