Healthy Change
Health is an easy thing to measure, but a hard thing to change.
Ford Health's unique Healthy Change Program is designed to assist individuals to achieve real and lasting behaviour change in relation to their own specific health risks.
The multifaceted program aims to motivate, support and educate individuals, and is structured to suit each individual's personal needs and motivations regarding their health. Over a 12 month period, individuals are regularly contacted by a qualified and experienced health professional, with personalised telephone coaching.
The Healthy Change Program is based on the Stages of Change Model, which utilises scientific psychological principles to promote health behaviour change (Prochaska & DiClemente 1986). The model recognises that change does not happen in one step. Rather, people tend to progress through stages on their way to successful behaviour change. Each person progresses at their own pace and this depends greatly on the support and motivation offered.
Behaviour Change Process
In each of the stages, a person is challenged by a different set of issues and tasks that relate to changing behavior. For each stage of change our approach varies, which is fundamental to the success of the program. Our Healthy Change Program is conducted one-on-one which enables us to customise our approach based on each individual's personal needs.
It aims to stimulate an individual's motivation to change though a carefully crafted style of counselling.
Along the way, relapses are common, where individuals revert to an earlier stage of change. The Healthy Change Program supports individuals through these difficult times with personal mentoring by a health professional. This minimises relapses and significantly increases the likelihood of achieving the desired behaviour change.
