1. Collection:
All information collected, either in paper format, computerised or verbal is treated in the strictest confidence by all treating health professionals and supporting staff and other staff as authorised by the Medical Director.

2. Use and Disclosure:
All information collected is solely for the use or the purpose for which the assessment or examination is conducted. Ford Health believes the collection of such personal information is fair, lawful and not intrusive to enable assessment and management of medical conditions as requested.

a) An assessment is reported solely to the individual.

b) An Insurance Medical only to the insurance company concerned (who will be responsible for the account) and marked "Private & Confidential" to the nominated person at such company. A copy of the pathology results will be sent direct to the individual.

c) For pre-employment, the Employer company concerned marked "Private & Confidential" to the nominated person at such company.

d) Health Promotion is reported to the individual.

e) Company Reports are reported as statistical data only with no reference to employee, condition or gender to identify any individual. If such a report is requested, then the requesting company will get written permission from all participants that such a report can be generated and supplied for future workplace management and enhancement.

f) Medical Consultations are treated on a strict Doctor/patient basis. Under no circumstances is any sensitive or personal information Disclosed or released to any person who is not one of the above unless ordered by a Court of Law. All such requests are referred to the Medical Director.

3. Data Quality:
Ford Health takes all reasonable steps to ensure that the personal information it collects, uses or discloses is accurate, complete and up-to-date by the regular completion of health questionnaires and verbal and written information as provided by individuals.

4. Data Security:
Ford Health takes all reasonable steps to protect the personal and sensitive information it holds from misuse and loss and from unauthorised access modification or disclosure.

5. Openness:
Ford Health abides by the National Privacy principals in relation to information it holds in regard to personal information.

a) Personal information is held as given by an individual by the questionnaire or consultation process

b) The personal information is stored and used for the sole purpose of accessing and managing an individual's current and future health risks.

6. Access and Correction:
Ford Health is happy to discuss and correct any information that is held by us that an individual advises is incorrect.

a) An individual can by notice in writing request a mutually convenient time to gain access to personal information that may be held by us.

b) An individual can lodge a complaint with us in writing as to the correctness of such information we hold.

7.Identifiers:
Ford Health does not adopt, use or disclose, an identifier that has been assigned by a Commonwealth Government 'agency'.

8. Anonymity:
Ford Health will give the option to interact anonymously whenever it is lawful and practicable to do so for the medical treatment of an individual.

9. Transborder data flows:
Ford Health does not transfer personal information to a recipient in a foreign country in circumstances where the information will not have appropriate protection.

10. Sensitive Information:
Ford Health collects sensitive information from individuals with their written consent only, or as required by law for the collection of health services provisions.
